Cancellation Policy
We try to call in advance to remind you of your scheduled appointment. We ask that if your are unable to keep your reserved time that you please call to inform us. A $50 charge may be assessed for any appointment that is not cancelled without twenty-four hours notice.
Insurance/Financial Policy
As a courtesy we will be glad to accept and file your insurance for you. We will give you an estimate of your payment amount for the services recommended. Upon your request, we can file a preestimate with your insurance company to determine what your exact out of pocket expenses will be. Your payment is due at the time the services are provided. Any procedures not covered by your insurance is your responsibility. Financing is available if you are approved through CareCredit. Visit their website to submit an application.
